Final registration will begin at 11:30 AM Friday, October 2, 2015. Refreshments will be available during registration. The program will run from 12:00 PM to 9:15 PM on Friday; and from 8:00 AM to 5:15 PM on Saturday and on Sunday. There will be 15-minute refreshment breaks midmorning and midafternoon. Lunch will be from 12:15 PM to 1:00 PM on Saturday and Sunday. Dinner will be from 5:15 to 6:00 PM on Friday. Luncheons and dinner are included with your registration.

The program will run from 8:00 AM to 5:15 PM each of the course days.
There will be 15-minute refreshment breaks mid-mornings and mid-afternoons.
Lunches will be from 12:15 PM to 1:00 PM The lunches are included in your registration fee.
If you have special dietary needs or are disabled and have special needs, please contact us at least two weeks prior to the program.
You will need to treat an implant patient in your practice between the two course weekends. You will need to: 1) treatment plan—be prepared to treatment plan at least one patient during the first weekend session, 2) complete—the surgical/prosthetic work between the first and second weekend sessions, and 3) present—the completed case or case in progress during the second weekend session. Six hours of credit will be given for your in-office work.
Credits & Attendance
Because credit is earned on an hour-per-hour basis, your credit will reflect the hours that you attend the program. However, if you are taking this module in order to complete the “Certificate Program in Contemporary Restorative and Esthetic Dentistry” you will need to complete all of the hours listed in the module. If you miss part of a module, you will need to make up the credits. Please keep in mind, especially when making your travel arrangements, that this module is offered on an alternating year basis.
What to Wear
We suggest that you bring a jacket or sweater to help you adjust to the seemingly inevitable changes in meeting room temperature.
A full refund will be issued if you cancel at least one month (30 total days) before the program. If you cancel less than 30 days prior to the program $595 will be deducted from your refund. For registrations charged to your credit card the refund will be credited to your account. No cash refunds will be issued on charged registrations.
Parking - October 2-4, 2015
Parking is available in the Washington Avenue Ramp. The Washington Avenue Ramp is located at Washington Ave. SE and Harvard St. SE. The Washington Avenue Ramp is located at 501 Washington Avenue SE, Minneapolis, MN 55455.
You will need to push the button on the stationary box in order to receive a parking slip and activate the gate. Detailed instructions for exiting the ramp will be provided during the program. Parking charges will be billed to Continuing Dental Education.
If EVENT PARKING is in place as you enter the ramp, please inform the attendant that you are attending a Continuing Dental Education conference in the School of Dentistry.
PLEASE NOTE: If you park anywhere besides the Washington Ave. Ramp, you will be responsible for the parking fees.
The Commons Hotel Guests Only: If you are staying at the Commons Hotel, we encourage that you make parking reservations directly with the hotel. We cannot validate parking for overnight stays. Validated parking is only available during course hours.

Hotel accommodations are available at the Commons Hotel, located directly across the street from the Dental School. To reserve your room at the special Continuing Dental Education rate:
- Call the Commons Hotel at 612-379-8888 and ask for in-house reservations. Tell the reservationist the dates of your stay and that you want a room under the reservation for “Continuing Dental Education.” Ask the reservationist to enter your name for one of the rooms at the special Continuing Dental Education rate.
- If you are outside the United States or do not wish to call, please email firstname.lastname@example.org with the dates you intend to stay and ask to have your name entered for one of the reservations under “Continuing Dental Education.”

Hotel Accommodations for January 21-23, 2016 - Carlsbad, California
A block of rooms has been reserved at the Carlsbad Inn Resort until January 13, 2016. This quaint Inn is located just steps from the beach in the historic, coastal village of Carlsbad, California.
Reservations should be made directly with the Carlsbad Inn Beach Resort: (760) 434-7020 or (800) 235-3939; 3075 Carlsbad Blvd., Carlsbad, CA 92008. Room rates are $142 per night. Carlsbad is accessible via air transportation from San Diego, plus a 30-60 minute transfer from the airport. Car rental is recommended for access to the Carlsbad Inn and to the Zimmer training facility. However, complimentary shuttle service will be available daily between the Carlsbad Inn and the Zimmer Institute.
