• Faculty/P&A Absence / Vacation Approval Form (NEW, please use this form)
  This is a new School of Dentistry form that will replace the Absence/Vacation Approval Form as well as the AHC 1704 Vacation request form. You will now be able to list vacation requests, absence without vacation requests, and give notification of meeting or conferences attending as part of a University appointment all on this one form.
