Becoming a Patient

For new patients to the School of Dentistry

At the University of Minnesota Dental Clinics we provide general and specialty dental care in association with the School of Dentistry's educational programs. Each year, there are more than 100,000 patient visits in the dental clinics which are located in Moos Health Sciences Tower on the East Bank of the Minneapolis Campus as well as the new University of Minnesota Pediatric Dental Clinic, Made Possible by Delta Dental of Minnesota, and University of Minnesota Physicians Dental Clinic located on the West Bank.

Adult patients - Student Clinics
  • Schedule an appointment

    Call 612-625-2495 to schedule an appointment to become a patient in our dental program. At this initial appointment a limited exam and other diagnostic aids such as x-rays may be ordered. Teaching cases are selected based on the needs of the clinical education programs. Therefore, not all people may be accepted. Please call or email umdentcl@umn.edu with questions.

  • Bring completed forms

    Prior to this first appointment, we will mail you a packet containing two forms to fill out: The Registration Form and the Health History Form. You can download these forms by clicking on them. Bring the completed forms with you for your initial visit on the 7th floor of Moos Tower. Also, please download this Release of Records Form and bring it in as well.

  • Treatment

    After your initial appointment, you will be assigned to a dental student. You will then be able to schedule an appointment at the School of Dentistry’s comprehensive care clinic for dental care. Patients needing specialty work will be referred to one of our graduate programs.

  • Payment options

    Fees are typically about 30% less than what is charged by private practice dentists outside the School of Dentistry.

    • The School of Dentistry accepts cash and credit cards
    • The School of Dentistry accepts CareCredit dental financing.

For children (ages 17 and under)

We have two separate clinics for treating children at the University of Minnesota:

  • Moos Tower Pediatric Dental Clinic
  • University of Minnesota Pediatric Dental Clinic, Made Possible by Delta Dental of Minnesota

Either clinic with mail you a packed containing two forms to fill out: The Registration Form and the Health History Form. You can download these forms by clicking on them.   • Moos Tower Pediatric Dental Clinic

    Pediatric patients will be seen by predoctoral dental students who work under faculty supervision on the 6th floor of Moos Tower Dentistry. Call 612-625-7171 to schedule an appointment.

    Fees are typically 30% less than charged by those outside the School of Dentistry.

  • University of Minnesota Pediatric Dental Clinic, Made Possible by Delta Dental of Minnesota

    This clinic serves all patients including medically, cognitively, or physically challenged patients. Staffed by resident and faculty practitioners, this hospital-based clinic provides access to all levels of general dentistry, including diagnosis, treatment planning, and implants. This clinic is located in the Park Plaza Building, 701 25th Avenue South, Suite 400. Call 612-659-4900 for an appointment

    Fees are typically similar to those charged outside the School.

    • This clinic accepts most private insurance plans as well as Medical Assistance and Minnesota Care.
Adult patients - Faculty Practice Clinic

Our Faculty Practice Clinic, is a state-of-the-art facility staffed by current School of Dentistry faculty. The Faculty Practice Clinic operates like dental offices outside the School of Dentistry and does not require a preliminary screening. Please call 612-626-3233 to schedule an appointment. Fees are comparable with private dentists’ office fees.

Adult patients - University of Minnesota Physicians Dental Clinic

The University of Minnesota Physicians Dental Clinic serves all patients including medically, cognitively, or physically challenged patients. Staffed by resident and faculty practitioners, this hospital-based clinic provides access to all levels of general dentistry, including diagnosis, treatment planning, and implants.
