Patient Forms


Before your dental appointment, we ask that you print off and fill in the following two forms: 

ADULTS 17 and older

OR

CHILDREN 16 and younger

Be sure to bring the appropriate completed forms to the Registration Desk on the 7th floor of Moos Tower on the day of your appointment.

Helpful Tips for your First Appointment

Parking: Parking is partially validated for the Washington Avenue Ramp only.

If the Washington Avenue ramp says, "FULL" proceed to the "Reserved Parking" entrance and tell them you have an appointment at the School of Dentistry - they will let you enter and park.  Important: Do not park in the Fairview hospital parking ramp: It is very expensive and we can not validate.


Photo ID Required: To verify and to protect patient identity a photo ID is required at registration.  Registration can not be finalized or a chart created without photo identification.  Remember to bring your photo ID to your first appointment

X-Rays: If you have a recent set (within 2 years) of full-mouth X-rays you may bring them to your appointment. We will evaluate the diagnostic quality of the X-rays and use them if we can.  X-rays must be on film, not digital, and not printed on paper.

Medications: Be sure to list the actual name of the drug, not just what the drug is used to treat, for example: Listing "Prozac" is usable information whereas "anti-depressant" is not.  Please note that we must have a complete list of your medications at your first appointment. If they are not provided at your appointment you will probably have to reschedule your appointment.
