Application information

Thank you for your interest in the Advanced Education Program in Periodontology at the University of Minnesota. It is always a pleasure to encourage individuals to pursue this fascinating field of dentistry.

You may apply directly to the School of Dentistry (August 1, 2015 application deadline for the class of May 2016).

The following items detail the procedures for admission to the Advanced Education Program in Periodontology:

Download and complete the Adobe Acrobat Reader version of the Application for Admission. Please also print a copy for your records as Adobe Acrobat Reader will not allow you to save the information. If you apply through PASS, you do not need to fill out an application to the School of Dentistry.

We encourage you submit your application in early July and will offer a one-week externship to highly qualified individuals (Top 1/3 of their dental school) and applicants with a great interest in periodontics.

  • Complete application form specifying name, colleges attending, dates of enrollment, history of residency, research and publications.
  • Submit official/notarized copies of: 1) BS/A diploma and transcript, 2) DDS/DMD diploma and transcript, 3) three letters of recommendation with one from dean of dental schools office specifying GPA and class rank.
  • Upon acceptance to the Advanced Education Program in Periodontology, additional materials will be required by the Graduate School at the University of Minnesota.

International applicants whose primary language is not English are required to submit a TOEFL (Test of English as a Foreign Language) score taken within two years of their application deadline. Applicants must achieve a minimum > 102 IBT scored to be considered by the admission committee.
